遠隔転移を有する膵癌に対する一次化学療法としては,ゲムシタビン塩酸塩が推奨される(グレードA)

エビデンス

    遠隔転移を有する膵癌に対する治療の目標は,延命と症状の緩和である。遠隔転移を有する膵癌に対する一次化学療法としてどのような治療が推奨されるかを検証した。

    遠隔転移を有する膵癌に対しては,従来はフルオロウラシル(5─FU)を中心とした化学療法やホルモン療法を用いた比較試験が実施されてきた。5─FUをベースとした併用化学療法の有用性は若干ではあるがbest supportive care(BSC)より優れていることがメタアナリシス1)〜3)(レベルⅠ)で証明され,膵癌に対する化学療法の有用性が示された。さらに,近年はゲムシタビン塩酸塩を中心とした種々の比較試験が行われている。



    1. ゲムシタビン塩酸塩単独と他の抗癌剤との比較(表4)
       ゲムシタビン塩酸塩単独と5─FUとの比較試験が行われ,ゲムシタビン塩酸塩群が,生存期間と症状緩和効果が有意に優れていたことが報告されている4)(レベルⅡ)。その後,マリマスタットやBAY 12─9566(いずれもマトリックスメタロプロテアーゼ阻害剤),exatecanがそれぞれゲムシタビン塩酸塩単独と比較され,いずれの試験においてもゲムシタビン塩酸塩群の生存期間が有意に良好または良好な傾向であった5)〜7)(レベルⅡ)。NSC─631570(Ukrain)を用いた小規模なランダム化試験(第Ⅱ相試験)では,NSC─631570単独群,NSC─631570+ゲムシタビン塩酸塩併用群がゲムシタビン塩酸塩単独群よりも生存期間が有意に良好であったことが報告されている8)(レベルⅡ)
    2. ゲムシタビン塩酸塩単独とゲムシタビン塩酸塩を中心とした多剤併用療法との比較(表5)
       ゲムシタビン塩酸塩は,他の抗癌剤とも併用され,ゲムシタビン塩酸塩単独との比較試験が実施されている9)〜26)(レベルⅡ)。最近,ゲムシタビン塩酸塩とシスプラチン,エピルビシン,5─FUとの併用療法(PEFG療法)やエルロチニブとの併用療法,カペシタビンとの併用療法(中間報告)ではゲムシタビン塩酸塩単独療法に比べ,生存期間を有意に延長したことが報告されたが,いずれの報告もその差はわずかであり,また併用群での有害事象発生頻度は高率であった12)23)26)(レベルⅡ)。そのため,両併用療法を標準的治療法と位置づけるには現時点では十分なコンセンサスは得られていないと判断された。また,メタアナリシスでは,併用療法が良好3)(レベルⅠ),プラチナ系薬剤との併用療法が良好3)27)(レベルⅠ),カペシタビンとの併用療法が良好とする報告があるが,複数の異なるレジメンを解析したものであり,特定のレジメンを標準的治療とするものではないと考えられた。国内ではS─1が第Ⅱ相試験の結果28)29)(レベルⅢ)保険適応承認となり,またゲムシタビン塩酸塩との併用においても第Ⅱ相試験の結果30)31)(レベルⅢ)良好な成績が報告され,現在第Ⅲ相試験が進められている(ゲムシタビン塩酸塩 vs. S─1 vs. ゲムシタビン塩酸塩+S─1)が,現段階で本剤を第一選択薬とする十分なエビデンスは得られていない。2007年,ASCOではゲムシタビン塩酸塩単独療法とゲムシタビン塩酸塩+AG─013736併用療法のランダム化第Ⅱ相試験32)(レベルⅡ)の成績が報告され,併用群の生存期間が良好であったため,第Ⅲ相試験が進められている。
    3. 5─FUを中心とした多剤併用療法の有用性を検証する比較試験
       5─FUを中心とした多剤併用療法は,無治療33)〜38)(レベルⅡ)あるいは5─FU単独39)40)(レベルⅡ)やエピルビシン単独41)(レベルⅡ)と比較され,その有用性が検証されている。少なくとも9本の比較試験が報告されており,うち3本の試験33)〜35)(レベルⅡ)において,多剤併用療法群の生存期間が無治療に比べ有意に良好であったことを報告している。いずれの試験も比較的古いものであり,試験の質の問題が指摘されている。
    4. ホルモン療法の有用性を検証する比較試験
       somatostatin,luteinizing hormone─releasing hormone(LH─RH),octreotide,flutamideが3つの比較試験により有用性が検討されている42)〜44)(レベルⅡ)。このうち,flutamideを用いた試験のみ,治療群が無治療群に比較し生存期間が有意に良好であることを報告している(50%生存期間:226日と120日)43)(レベルⅡ)
       以上の結果から,遠隔転移を有する膵癌の一次化学療法としては,ゲムシタビン塩酸塩単剤治療が推奨される。

明日への提言

ゲムシタビン塩酸塩単剤治療を生存期間において上回る併用化学療法も報告されたが,標準的治療と位置づけるには十分なコンセンサスが得られていない。国内外で新薬を用いた臨床試験が進められており,今後の展開が期待される。
